As a new patient to SIHF Healthcare, please bring the following documents to your first appointment:

To obtain financial assistance with your co-pay or if you have no insurance, you can apply for our SmartPay program. 

If you do not have insurance, you may wish to speak to one of our Health Insurance Navigators or ask any patient registration clerk about applying.
