The SIHF Diabetes Care & Management Program is designed to help patients manage their uncontrolled Diabetes thorough access to care, specialists, and education.

 

Who Qualifies?

Patients with hospitalizations and ER visits related to uncontrolled Diabetes

Patients who have Medicaid (WellCare or Meridian) insurance

Patients who reside in the Metro East area

 

What does this program offer?

Diabetes care from their provider and Washington University specialists

Diabetes education & Nutrition Planning

A personal Continuous Glucose Monitoring (CGM) device – at no cost to you! 

 

What is a Continuous Glucose Monitoring Device (CGM)?

A continuous glucose monitoring (CGM) tracks your glucose levels throughout the day and night and can alert you if your levels go too high or low. Say goodbye to finger sticks!
