Southern Illinois Healthcare Foundation (SIHF) provides quality medical care to
all patients regardless of their ability to pay. An affordable and easily managed
sliding fee scale is available to patients who do not have medical insurance. This
fee is based on the income of an individual or family. Once proof of income is determined,
SIHF will adjust the payment owed by the patient. For questions regarding this policy,
please contact your local SIHF Health Center.
The following are examples of documentation preferred by SIHF to show proof of income:
- Prior or current year federal tax returns
- Copy of employee pay stub
- Copy Unemployment stub
SIHF accepts most forms of medical insurance. The following are a list of the most
